TRANSFER CHECKLIST

Prior to transfer ensure that:

  • If there is a likelihood that the patient will place himself or others at risk during the transfer, that the appropriate level of chemical or physical restraint is employed
  • The patient is accompanied by the:
  • Form 1 - Detention Order (to be given to the treatment centre)
  • Form 6 or 8 - Request for transfer (to be given to the treatment centre)
  • Form 15 - Request for transport (to be given to the ambulance/police)
  • Clinical documentation and nursing chart
  • Drug chart (if transferring to Brentwood)
  • A record is noted in the case-notes and HAS tracking system of where the patient was transferred to
